What is the bird fed on?  Include any treats given.

Is the bird sprayed regularly with water?

Does anyone smoke in the same room as the bird? If so, give details. 

Details of any training.  What requests from you, if any, does the bird obey?

Can the bird fly?  Does it fly well or clumsily?

Bird’s history.  Was your bird captive bred, hand reared or wild-caught?

Who gets on best with the bird?  Does the bird dislike anyone?

Is the bird tame, or nervous?  Will the bird take food from the hand?

Describe the bird’s general character; aggressive, bold, nervous, lethargic, etc?

How much time does the bird spend out of its cage each day?  Is it ‘supervised’ while out?

State what the problem behaviour is and when it first started. If plucking or screaming, does the bird do this when in or out of the cage? In your company? If plucking, which areas are plucked?

What do you do when you see the bird doing the unwanted behaviour?

Have you had other advice regarding your bird’s problems.  Was this effective? 

Are you familiar with Applied Behaviour Analysis in behaviour modification?
